Avenue at Medina Care and Rehabilitation Center

This facility takes excellent care of my relative. I am thrilled with the responsiveness of the staff at this facility and my relative takes a great deal of care. Even on days when they are short-staffed, they have been responsive and caring. The facility is clean, the food is good and hot when delivered. My relative was previously in a nursing home with a higher rating but it was horrible. The staff at this nursing home has been attentive to every medical change and reacted appropriately. I am very happy with the care at this facility.

My experience at The Avenue, was great. Everyone pleasant and kind. The building is clean. The food was good, with many options. Therapy and activities were also excellent. I have already recommended The Avenue.

Staff is slacking turn on a call light at 4:30 and don’t answer til 7:30 they said change of shifts couldn’t get a shower for 2 weeks and said that mom said no and it’s all about the way you ask them and my dad told them she needed a shower and they ignored him and she was in the hospital for about 3weeks and she only had 2 shower the whole time she was there and she was the for about 1month so sad ☹️☹️ I work in the nursing field and it was sad

The Avenue at Medina is beautiful and the people there are so friendly and welcoming. Housekeeping did a great job keeping my room clean and caring for my laundry. The nursing staff went above and beyond to make my stay here great after my knee replacement. I highly recommend the Avenue for anyone who needs care. The staff there made sure my incision stayed clean, and even provided transport to me for my follow up with my surgeon. Therapy worked at making sure i progressed safely to walking again. I can't say enough about the team there from start to finish. They did great at making my transition home as painless as possible, working together to make sure i had home health care, and medications at my pharmacy the day I left. Medina is lucky to have a facility that cares for our community as much as the Avenue does.

Choosing a care facility for my mom was a tough decision. I toured several rehab facilities in area and The Avenue immediately stood out as the clear winner. The positive impression started with representatives like Jordan, who clearly outlined their commitment to resident care and vision for the community. I personally spent a lot of time with my mom while she was at the Avenue and the excellent service that began on day one and never wavered throughout her entire stay. The quality of care provided by the team was solid. The nurses, aides, and therapy professionals were highly skilled and genuinely compassionate.
