Very nasty dirty facility nasty Hr an DON HIGHLY WOULD NOT RECOMMEND
My mom has been there for 2 in a half days. Nobody never answers the phone. There are no prompts or extensions, to get you through to the units. She won’t be there long.

Absolutely disgusted with the care my mother is receiving there. Will be filing a complaint with the OH Department of health and this place will be under investigation soon. Staff is very rude and doesn’t care about the patients at all. Degraded my mother in every way. If you care about your loved ones, think again before sending them here!
Horrible experience from beginning to end. The facility was old, outdated and completely filthy. Unprofessional and rude staff seemed they couldn’t care less about our loved one. Several other patients just walked around freely in and out of our room, fighting with each other and causing disruptions to where our loved one couldn’t get any rest. I wouldn’t send my dying dog to Momentous but unfortunately one greedy family member who happens to be the power of attorney forced them to go here and left Momentous to their care but only 3-4 days later they were dead. I’d give ZERO Stars if I could!
Being that this is the first nursing home that I have been in it truly has turned itself around with the new management. I appreciate everything that has been done and the things they are continuing to do to turn this place around. Great job staff!
This place has come a long way, in a short period of time. With new staff and new management it’s made a complete 360. Come in and see for yourself.
Very disappointed. Everytimr I vist it smells like poop. The residents pay 10000 a month and they can't fix the door bell or buzzard you call and wait 20 minutes at the door No one is answering the phone..the food here is terrible..I take my family out to eat he has lost so much weight being there....these nursing homes don't care about the patients or staff
