My loved one, who has since passed away, was previously a resident at this nursing home. Looking back on his time there, I remain deeply disappointed by the conditions and level of care I observed. Cleanliness was a major concern. There were frequent unpleasant odors throughout the facility, and the overall appearance of the building often seemed dirty and poorly maintained. The environment did not feel clean, comfortable, or welcoming for residents and visitors. During my visits, I was also bitten several times by what appeared to be insects while sitting in his room. This added to my concerns about the facility’s cleanliness and upkeep. One of my biggest concerns was his personal care. On numerous visits, I found him severely soaked in urine, with clothing and bedding that needed immediate attention. It was heartbreaking to see him in that condition. No resident should be left sitting in urine, and basic hygiene needs should be addressed promptly and consistently. I was also concerned that he was almost always in bed when I visited. It seemed that he spent very little time out of bed, which raised concerns for me about his overall care, activity level, and quality of life. I became increasingly concerned about whether he was receiving adequate assistance with meals. On several occasions, I arrived to find his meal tray sitting untouched on the bedside table, positioned out of his reach. He was unable to access his food without assistance, and seeing meals left where he could not reach them was extremely upsetting. As a result, I often planned my visits around mealtimes so I could make sure he was able to eat. I was also concerned about staffing. Staff often appeared overwhelmed, and when I raised concerns about his hygiene and need to be changed, I sometimes felt that my concerns were treated as an inconvenience rather than a priority. Residents deserve compassionate, respectful care and to be treated with dignity at all times. Based on what I personally witnessed during his stay, I believe significant improvements are needed in cleanliness, staffing, resident hygiene, meal assistance, and overall quality of care. I would not recommend this facility based on my experience.

Dumps patience at homeless shelters after any financial issue and not even complying with 30 days notice laws. If you show up for your loved ones and can check on them regularly they're probably fine, but if not I'd highly suggest against them. They don't actually care about people, they just care if they're getting paid.

Our fault for not reading reviews before sending my grandmother here, but please believe the other negative reviews given to this facility. Not everyone here is bad, I will say. But, unfortunately there is enough bad to outweigh the good. Telling an elder that her disability is not true and to do things she cannot do on her own is never okay. If you do not have compassion, do not work in a nursing/rehabilitation facility please. Truly not sure how the place is up and running.

Only pleasant experience my family had with these people was with the social worker. She is great. But other than that don’t send your loved ones here

If you love your loved one, you would never let them step foot in here.

I like that Concord care is located in a beautiful rural setting away from the hustle and bustle. I have witnessed staff function as a team to deliver quality, skilled nursing care to the residents with compassion and respect. There are some agency employees but the core staff and DON work together to make sure everyone is on the same page. The Administrstor is compassionate and concerned with the welfare and quality of life of each and every resident and works with the families to make the transition to long term care as easy and comfortable as possible. The residents receive skilled therapy services as needed from the on-site therapy providers which they seem to really enjoy. They have an activities department who keeps the residents occupied, entertained and informed of current events. Also, the food is delicious! The families of the surrounding community are blessed to have the local skilled care facility of Concord of Hartford, their loved ones can call home.

Everyone is very polite. I like the activities that they do with the residents. The Admission Director is very knowledgeable. I will recommend this facility to family and friends.

I would give zero if allowed. My mother died in this facility shortly after her stay. The staff was mostly agency workers and many had no idea who my mother was or even her level of care needed. The center didn't want my mother to have Youngstown Hospice care to care for my mother as Concord Care wanted their own. I fought to have Youngstown Hospice come in to help. It was approved too late. Her death was traumatic for our family and hospice of Youngstown was our only ray of hope to help us through that time. Concord administrators never spoke to me during or after my moms death....not even being sorry for her loss.
