My father was placed at the facility to strengthen him up. He was on a pic line, just had bowel surgery resulting with an illestomy and a catheter. They were informed about our concerns regarding infection right away. He did not receive a shower for a month. Over the course of his 2 month stay staff gave him 2 showers. I gave him the rest. He developed a uti in there which took over a week to get him on an antibiotic. He has kidney disease already so what’s the problem. When they decided to take him off the tpn I expressed my concern that it was too soon because he was barely eating to the doctor, staff etc. No one listened and I was assured they would have a nutritionist watching this. A couple weeks later they discharged him. I took him to my house and he was acting weird. He had no urine anywhere in the tube. Called the facility since I had them change the catheter and was told it probably needed flushed. Took him to the ER and informed them of the problem and told them I thought he was dehydrated and probably malnourished. Learned he was in kidney failure, severe dehydration, potassium level of 8.4. All of these affect the heart and he already had heart failure. And to add to it malnourished. He was hospitalized. No one should go into a facility and come out worse. I believe this has caused further damage to his heart and kidneys. Totally pathetic!!!! And to think he was charged 300 dollars daily . For what ? The following photos show my dad in March at the facility then 2 days after discharge

Great place to live
My husband has two family members in Countryside Manor. They are very well taken care by the nurses and nurses aids. They tell me that themselves. And I appreciate that. But when a resident is being moved in an empty room. The room should be deep cleaned first. Not just swept. My husband's uncle was moved just two rooms down the hall and the new room was dirty and five minutes later another resident moved into his old room with not cleaning it at all. It was just swept. I know how to clean a nursing home because I work in one myself. If I didn't deep clean before the resident moved in, I would be in hot water. So please before a resident moves into a empty room deep clean it. It will prevent many infections for everyone especially the residents.
Another Five-Star Review for Countryside Manor – Fremont, Ohio I just returned from visiting Countryside Manor from June 9th through June 13th, and I’m filled with so much gratitude. This was my first time seeing the facility in person since my son was transferred there on Mother’s Day—and let me say, it was everything I hoped for and more. The care they’ve provided my son has been outstanding, and seeing it for myself brought me so much peace. The facility is clean, organized, and filled with kind-hearted people. From the moment I walked in, the hospitality was warm, genuine, and comforting—just as I expected after all the communication and support I had received from afar. I want to say thank you from the bottom of my heart for the excellent care you’ve shown my son. Being so far away from home in Jacksonville, Florida, has not been easy, but knowing he’s in such capable, loving hands makes all the difference. If you’re looking for a place for your loved one—especially when distance separates you—I highly recommend Countryside Manor. God bless each of you for the work you do. Keep shining and keep up the great work. With heartfelt thanks, [A Grateful Mother from Jacksonville, FL]
Five-Star Review for Countryside Manor Healthcare – Fremont, Ohio My son was transferred to Countryside Manor Healthcare on Mother’s Day, all the way from Jacksonville, Florida. It was incredibly difficult and heartbreaking for me, especially since this facility is so far from home. I was disappointed and deeply saddened that no long-term care facility in Florida was able or willing to take him due to his young age and the type of care he needs following his brain injury. Countryside Manor was the only facility that said yes. At first, I was strongly against him being moved so far away. But after speaking with Brooke, the most amazing case manager, my heart began to soften. She reassured me with such compassion and confidence that they were ready to care for him properly—and that they would do right by him. Now, three weeks later, I can honestly say: they’ve done better for him than any facility he’s been in so far. From the nursing staff to the caregivers, the difference is like night and day. Special thanks to Darlene, the Director of Nursing. Her passion and dedication shine through, and she helped me feel at peace knowing my son is in loving hands. Until I can make the trip to see him in person, the FaceTime calls they’ve allowed have meant the world to me. I’m counting the days until I can visit and give these incredible people the biggest hug. Thank you, Countryside Manor, for treating my son like family. You are a blessing, and I proudly give you a full five stars. Keep up the amazing work. With deep gratitude, [A Loving Mom from Jacksonville, FL]
Even though some of the AIDS were helpful and friendlyThey were understaffed and my mom did not get the care and dignity she deserved She was bed bound and legally blinnd She was lucky to have a shower or get her teeth brushed once a week She needed help to eat but didn't get it When I came to visit her clothes were soiled in food and sometimes find food particles in the fold of her neck She died there last month..coroner said pneumonia
I’m extremely grateful and impressed for the way this facility take care of people‘s families, the elderly and disabled. Hats Off
