I am extremely concerned about the unprofessional and unsafe environment at this nursing rehabilitation center. In addition to concerns regarding the quality of patient care, I have personally witnessed nursing staff being extremely rude, hostile, dismissive, and unprofessional toward patients and their families. Families should not feel intimidated or uncomfortable when asking questions or advocating for their loved ones. Staff members should communicate respectfully and professionally, especially when dealing with vulnerable patients who depend on them for care. There also appear to be serious concerns regarding staff competency, patient hygiene, cleanliness, and whether patients are receiving the basic medical care they require. These concerns are serious enough that I believe the facility needs an immediate investigation by the appropriate regulatory authorities, including the Ohio Department of Health. I am requesting that these allegations be investigated thoroughly, including the conduct of the nursing staff, the quality and timeliness of patient care, staffing practices, cleanliness, and overall patient safety. If violations are found, immediate corrective action should be taken to protect the patients at this facility. Patients deserve to be treated with dignity, respect, and appropriate medical care. Their families should not have to fight to ensure that basic standards of care are being provided.

This is the most horrible place Ever get your family out of there this place needs to shut down not clean nurses need to go back to school so unprofessional

I am extremely concerned about the quality and safety of patient care at this facility. There appear to be serious issues with how patients are being treated and how basic nursing responsibilities are being handled. Patients are not consistently receiving appropriate care or attention. In some situations, patients are informed of important information by nurses rather than receiving proper communication through the appropriate channels. During Bible study, patients have reportedly been locked outside, which raises serious concerns regarding their safety, supervision, and dignity. There are…

There are also concerns regarding basic hygiene and patient care. Patients are reportedly not being cleaned properly, and some patients are left in soiled diapers for extended periods of time, including reports of patients being left for more than eight hours without having their diapers changed. I am also extremely concerned about the competency of some nursing staff. There have been situations where staff members appear unable to perform basic medical procedures and nursing tasks that should be within their scope of training and responsibilities. Additionally, there are serious concerns regarding possible substance use among staff. It has been reported that numerous nurses may be working while under the influence of drugs or alcohol. These allegations should be investigated immediately because impaired healthcare workers can place vulnerable patients at significant risk. Most concerning, there are patients who are experiencing severe mental-health crises, including patients reportedly attempting to harm themselves because of their medical conditions. These patients require close monitoring, compassion, appropriate intervention, and a safe environment. I am requesting that these concerns be investigated thoroughly and promptly. Patient safety, dignity, hygiene, medical care, staff competency, and the possibility of staff working while impaired should all be reviewed. I believe these issues warrant immediate attention to ensure that patients are receiving the safe and appropriate care they deserve. Do not send your family or friends here ever ! If something doesn’t change I will be calling the Ohio department of health, the Abuse Neglect, misappropriation and exploitation program and I will be calling adult protective services I have reached out to the ombudsman and nothing is getting better!

This place is jail for residents they understand staff and don’t care bout the patients, they are rude and it’s disgusting

I have been here for roughly 2 months and there are good and bad things about the place. --------------------- As for the negative things: Administration is really REALLY judgemental. Especially towards addicts. They restrict visitation if you are prescribed any form of M.A.T. (Medicated Asisted Treatment) — even if you're there to rehabilitate from major injuries. They also like to make up rules as they go and do not distribute the "rules" evenly towards every resident. There seems to be a lot of illicit drugs distributed between residents to other residents. Most of the residents who are involved in it are not even a part of the harm reduction group that is an out sourced agency called Stepping Stones. I will say that the counselors and the Stepping Stones supervisors genuinely seem to care about their clients and they notice that administration will often times directly target residents who are clients of the program. I have had my counselors try to advocate for me on multiple occasions to the administration about some of the restrictions that they recently started to implement towards the residents who are clients of Stepping Stones but they do not implement the same restrictions to other residents who are taking part in substance abuse or distribution– therefore, the problems end up not getting solved. Also... —BE ADVISED THAT THIS LOCATION RECENTLY BEGAN ACCEPTING *SEX OFFENDER* CLIENTS— When I first arrived here, this was not the case. However, about 2 weeks ago, I was given a written notification from the administration that a "non-violent sex offender" had been admitted to the facility. And then yesterday, I received yet another written notification that another sex offender had been admitted. They plan to continue accepting "non-violent sex offenders" for the foreseeable future as they have now cleared out an entire hallway upstairs to house them separately.... although there will not be any type of security on site AT ALL. Another extremely frustrating issue is the dietary department. Not only is most of the food not very good, they claim to offer "alternative" meals on a daily basis. (Cheeseburgers, grilled cheese, hot dogs.) However, the majority of the time if you ask for an alternative, they will just put you off and sometimes even go as far as get verbally offensive to your nurse aide who tries to advocate for you and get them to give you the alternative meal. I have even had a kitchen worker deny me the alternative right to my face yet happily give it to another resident sitting directly across from me. Lastly, do not expect to ever see the facility's head doctor in person. The entire time I have been here , I've only seen him once. And this is the doctor who is in charge of ALL the medications you are prescribed and for your genereal health during your stay there. You can ask your nurse to pass along messages to him for you... but you'll never be able to talk to him personally. ------------------------------- Now, as for the good things about the place: Even though the majority of the nurse aids are pretty lazy and put you off when you hit your call light... there are a few aids who are just absolutely fantastic!!! (Lashawna is my absolute FAVORITE nurse aid!) The counselors from the out-sourced substance abuse/harm reduction program called Stepping Stones are all just reallyyyy kind and helpful. They really make you feel like you can just be honest with them about anything — good or bad. They also make the group therapy sessions feel like a very safe space where you can share without judgment. --------------------------- As a whole, I probably wouldn't recommend this place to anyone. 🫤

This place is disgusting! I wouldn’t send a dog to rehabilitate there. My grandmother was sent there to get rehab from Covid and was mistreated and neglected. It took me weeks to get her transferred. Please do not let doctors send your loved ones to this dump.
