The staff is lazy except for a few staff.

Hello my name is Orlando McCutcheon. This place is incredible the staff the smiles I was broken when I came here. And now I'm going home with a new attitude and a new just just grateful I am truly grateful Orlando McCutchen

My dad was here and the staff treated him bad they try to keep him medicated and say that he was combative. Funny thing is we moved him to a different facility and he has improved and doing much better. He eats and enjoys the activities. No wonder they are a 1 star in Ohio DHS

This is a horrible place dont send your family here. The DON gerri was in her office with her husband violating hipaa. I was trying to speak with her about my dad and she had all his paperwork on her desk in front of her husband probably getting his info. They are understaffed and lost the VA contract cause this place is horrible run.

My father was in this place for a few years, to me his health only declined here, he ended up becoming tube feed while he was at this facility I urged for rehab to eventually elevate his diet to food again, however, time after time they told me nothing was improving. They told me that my father was difficult to do rehab with sometimes because he would decline. I moved my father into a new facility and within two months his diet was able to be upgraded to puréed food and he is no longer tube feeding additionally I can just tell overall my father was not being taken care of and attended to while he was in northwestern from cleaning and personal care to rehab services, it’s a reason the VA is no longer working with this facility, the building was clean and employees were nice only pros I can think of

Dedicated staff. Clean facility. Therapy team is wonderful.

Horrible place. No wonder they have lost their funding from the VA. My father was abused while under their care and they never reported it to the state. Instead of firing staff that was accountable for these actions they transfer them to other facilities run by their communicare corporation. So be aware they have other facilities in the local area. Over the year while my father was here we witnessed medications laying on the floor. Rooms are not cleaned, even after days of repeated notification to the staff. Nursing staff is constantly on their phones instead of working. Phone in room was ripped from the wall so my father was unable to have contact with friends and family who he would normally have contact with. Along with the call button to reach nursing staff. Administrators Endia and Geraldine Tigges are not honest caring people. They only try to cover up for their staff and lied on a police report that was made involving the abuse with my father. Do not leave your loved ones here if you care anything about them. This place should be shut down permanently by the state

My sister is staying at this nursing center and I am very grateful for the Executive ,Endia Afzal. She is kind, accessible understanding with a very genuine concern for the proper care of the residents at this facility. She gives me hope that my sister will be treated well and kind and she gets the proper care for the greater good for her well being while she is at this facility. I am very grateful for her. And the nurse Michelle that is there sometimes really is professional , kind and respectful to my sister and it would be great if she was the head nurse. - Dianne R.
