This facility is awesome treated my wife Mary milliman like family thanks to office staff and all the nurses and aids especially Henry he stopped in yesterday after she passed and said his goodbyes the hospice staff that came in were awesome to
Horrible food, dirty room, amateur staff.
Impressed with the cleanliness of the facility. The staff members are very engaging and are very professional. I’m impressed with the upper management especially the executive director in terms of having a good understanding of the facility and the rest the ED has from the staff
Excellent care for Dad. All the staff is helpful, kind and always eager to assist. Simply the best care in the area.
During my mother's week-long stay at the facility, I witnessed extremely poor service. The staff treated the residents, many of whom are older adults who deserve a higher quality of life, with disregard and disrespect. For example, one day while I was visiting, my mother, who is blind and unable to serve herself, asked for water. I pressed the help button and waited for almost 30 minutes, but no one came to assist her. I tried pressing the button again, and still no one responded. Eventually, I went to the nursing desk to inquire whether the button was functioning. The staff member told me that the light on the dashboard was on, yet the nurse was preoccupied with texting on her phone. This experience highlighted the inadequate care provided at the facility.
My husband was in the Fairview Park facility because he needed less aggressive PT, came in late on on Monday evening, on Tuesday, he was left on a bedpan, in his own waste for over an hour and a half, no one came to clean him up, I DID. The aide said she forgot him. I left 2 message for the administrator Ana to call me, I have yet to hear from her. On Wednesday, when my husband had one of his bad headaches, they sent him to Fairview Hospital because they didn’t know how to help him. Stay away from this facility!!!
This place has been amazing. The quality of care and compassion that they provide is above and beyond.
My grandmother called my cellphone at 7:13 PM in a panic because she couldn't breathe. Her oxygen tank from her afternoon dialysis appointment had run out of air (it's unknown the exact time the tank had run out of air) and when she realized she called me in a panic because her oxygen was restricted and she was suffocating, gasping for air. She yelled out in the hall but none of the nurses in the facility responded. I called 911 (paramedics) at 7:14 pm and made them aware of the situation. The paramedics were able to get in touch with the facility and had called me back at 7:22 pm and the paramedics noted the staff at O'Neill kept redirecting the call and it was hard to get in contact with them to ensure my grandmother's safety. I arrived at the facility at 7:35 pm to ensure my mother's safety, and by the time I got there my grandmother was put on the oxygen concentrator. I'm unsure how long my grandmother went without oxygen and no one provided me with a timeframe. When I arrived at O'Neill I spoke with Lisa, the LPN for the evening shift and asked her a couple questions, I also recorded the conversation I had with her for evidence. I also asked who her assigned nurse was after she arrived from dialysis because it's he duty of the nurse to ensure life threating incidents like these don't happen. Lisa informed me Megan was the name of the nurse assigned to my grandmothers room. Megan didn't do her job and put my grandmothers life at risk. Who knows when if my grandmother would have recieved help if I didn't call 911.
