My mother's care team has been wonderful. Liz, LPN, Destiny, STNA , Sheyenne, STNA are without a doubt the dream team! Excellent care from them every shift they are on. Our family feels blessed that they are providing the care they do. Not enough thank you's can be given in this lifetime. -Heather

I am so upset. Went to see my uncle Joe Fowler and as I got near his room I could hear him yelling for help because he was almost out of bed . He still has a good mind after his stroke. I went looking for help and found all nurses and aides setting in nurses room . Unexceptionable. He is a decorated veteran. He has had no bath . Hair is dirty. Bathroom was dirty. I was going to help clean him but bathroom was dirty. He has not been taken care of.

TERRIBLE! DO. NOT TAKE A LOVED ONE HERE! We have a close family friend living here & she has experienced this herself & her roommates & surrounding room patients...letting people lay on the floor for 45 minutes after falling & calling for help, not helping those out of bed to go to the restroom for an hour and then laying in their soiled bed after call lights have been hit & left unattended for an hour. A couple good gems but majority of employees here could care less about the patients. I wouldn't send my dog to this facility. I feel terrible for all these people living here. It's so sad &…

DON’T BE FOOLED During the very short time my mom was there, we had many incidents that were incomprehensible and had to call for a care team meeting. For a patient who was oxygen and bi-pap dependent, she was found multiple times with her oxygen tubing not connected to the concentrator or her bi-pap not connected properly. I found her asleep with her bi-pap machine not functioning at all. I was told by the PT she had just been in 10 minutes prior and it was running so my question was since my mom couldn’t get out of bed to turn it off and no one else says they had been in to turn it off, how many times a night does it quit working and nobody notices and she goes without bi-pap support? I was assured at the care team meeting that since it was under warranty that it would be replaced immediately but they never did replace it for us. Another incident was that my mom was struggling to breathe one afternoon and asked for help putting on the bi-pap machine and a nurse flat out told her no and to quit asking during the day because it wasn’t her job to help her put it on during the day, just at night - because that’s what the chart said. So instead of helping a patient in need or addressing the issue with the doctor she just let my mom continue to struggle. Staff would bring in her food tray and put it on a chair across the room instead of on her tray table, even though she couldn’t get out of bed by herself to get to it. And if she by chance she was still asleep when they came through to retrieve the trays they would try to take it away without her getting a chance to eat it and we would have to tell them “its’s still a full tray, she hasn’t eaten yet”. How many meals did she go without when no one was there? At the care team meeting staff seemed to blow off our concerns. The nurse manager kept telling us about how “when she worked at Hospice” and kept saying “your mom is a very sick lady” as if I didn’t know this already, almost like she was just waiting for her to die. I don’t care how sick a person is, it should not affect the standard of care a patient deserves. When she was finally sent to the hospital she had been sitting up in a wheelchair and was waiting on PT, when PT came in they called for a medic for transport to ER due to mom being in an altered state, the staff said she had been like that all morning. (How long would it have taken them to do something? If ever? So thankful for PT) Once she got to ER she was in very grave danger: very low blood pressure, extremely high blood gas level, severe UTI, pneumonia, severe bowel blockage (they were aware of her bowel issues upon admission and were told that she needed Miralax daily but they would not adress the issue even after multiple requests). While in the hospital before she passed she asked please don’t take me back there to those mean people. After my mom had passed away at the hospital family went and cleared out her room. During the next week I had even went in and stopped at the front desk to make sure everything was taken care of and talked to the receptionist and was taken back into someone’s office. But then one week after she passed I got a call from the same receptionist stating that my mom had been vacant for more than 48 hours and asking if I wanted them to continue to hold her room?—— Really? That’s how yow you handle a patient’s death? This place has a five star rating, don’t be fooled, there is no way on earth that they provide five star care. I chose them based on their great ratings. Boy, did i make a mistake.Physical Therapy was the only positive thing about this place. While although I cannot say that they caused my mother’s death, in my opinion I am 100% certain that they are responsible for expediting it by there lack of care and concern.

My father has been at Stonespring in Vandalia for the past week, and the rehab and care he has been receiving has been wonderful. Great communication and a special shout out to Destiny, Wendy, Derek, Brittany, Dana and Celeste! It is great know he is with such a caring staff. Thank you!

DO NOT BRING YOUR FAMILY MEMBER TO THIS PLACE FOR ANY KIND OF REHAB!!!! I continuously told numerous people about swelling in my loved ones leg only to be ignored. I called daily, went nightly. Never received a call back until I put forth action and called the surgeon whom did the surgery on my loved one. They took Staples out 5 days too early never check the site again. After I contacted the surgeons office I was told to bring him to the ER. So I brought him to the ER. Upon arrival they realize my father had a terrible infection where the sight of the Staples were he was hospitalized for 5 days. Do not bring your loved one here unless you want them to go through pure hell. The staff is unbelievably rude and inconsiderate. There was only 1 nurse who was exceptional and I was sure to tell her!!

I love this place! its like a beautiful castle on a hill! Everyone is so friendly and welcoming. Food is fantastic and therapy is amazing. So many of the nurses have been there for many many years.. Something you don't see often anymore. They have a really great team from top to bottom.

Avoid this place at all costs. The physical therapy and occupational therapy staff is great, but some of the nursing assistants are absolutely awful. One of my nursing assistants refused to put the gait belt on me when I had to get up, she also refused to wear face masks when Covid was going around the facility (which was required of all staff members). My roommate fell one night so I had to yell for help because they were so late and getting to the room he fell and he was bleeding and the same assistant told me I was never to call for help ever again, but her manager called her out on that when they got a statement from me about what happened. Hopefully they fired her. She's she's incompetent as a nursing assistant. I had to pay out-of-pocket for a two extra weeks before I could be released, and they billed me for it, (which I agreed to,) but they screwed up the bill not once but twice and now they're sending me a letter saying that in'm late on the payment and their attorneys will contact me. I paid an agreed-upon 1/4 of the $7,600 bill (other portions to be paid over the next three months). I paid $1,900 on September 21, but they billed me a late fee on August 26...the day I was released. They also had the dates of my extra days totally wrong. their accounting department is incompetant, in my opinion. DO NOT STAY HERE!!!!!!! Furniture is outdated and run down...if they have a lot of residents, they'll put you in group therapy wih abou 10 or more people...you're being charged for 45 minutes, but you'll only get about 10 or 15 minutes of on-on-one rehab. Meals are good, but many times as much as 45 minutes to an hour late being served in the dining room...while everyone sits there waiting. As mentioned,Katie, Josh andAllie (PT an OT therapists) are very good, but limited in terms of space and equipment.
