Thia Thia facility was amazing. The care that was given to me was exceptional. Crystal the aide especially was great as well as Makayla. mike and carol in PT was awesome
My mother needed to be admitted after spending a week in the hospital. Her legs were weak and they felt physical therapy would help her. The facility is very clean. It smells clean and not like you would think a rehab facility would smell. The staff were all very friendly. Our STNA Tia was the most caring, attentive, patient, devoted, hard working person. She cared for our mother like she was her own. She reassured us that our mind could be at ease when we were not able to be there. She took extra good care of our mom and went the extra mile. We would recommend this facility to anyone in need…

Crystal and Sheronne are excellent aides working in the hall where my wife is a patient. They treat my wife with great care.
My dad broke his hip and was not very easy of a person to care for with his age of 88 years. Tia was his nurse aid and she went well beyond her duty caring for him and encouraging him to do more. I do believe because of her it helped him to get stronger more quickly. Your company is fortunate to have such a dedicated young lady.
Living through the roller coaster of ups and downs with my mother’s PLS and recent surgery, but Crystal, Paige and Aida in particular have been so gracious, patient and caring with Mom’s needs. They are who this review is for. With so many hardships and challenges, those little moments when she gives them a thumbs up are very meaningful. We appreciate them!
NEVER SEND YOUR LOVED ONES HERE. My grandmother came here after a fall. She needed assistance to use the restroom so she would press a button to turn on her light outside the room to request assistance. 5 minutes later a nurse would pop in to say “I’ll be with you in a minute” and turn off that light so they don’t need to return. The quickest service she ever got was 20 minutes, and the longest she sat in her own urine and feces was 90 minutes. When she would relieve herself in her diaper after no assistance, the nurses would get upset with her. Meanwhile the staff is at their desk huddled around a phone watching a movie. Needless to say we removed her and will never go back. The only redeeming quality was the PT, which is subcontracted, not done through the facility. Place looks beautiful on the outside, but it is terrible on the inside. Send your loved ones somewhere else
The Concord Village is a decent facility: clean, staffed by some caring, attentive staff who have the patient's needs in mind. There are food options. PT is very is very comprehensive, prompt, and cooperative---working together. The grounds present attractive scenery from every room.
As with most reviews I too only gave one star because you can't give a 0. We just removed our family member from this place. Where do i start? Although the place was clean, there were bugs inside and had too laugh as i watched a cricket make its way across the hall as 6 nursing staff personnel walked by, seen it and kept walking. I watch as a staff member offered my family member her meds and she (with dementia) said "no" and they said "ok, i tried" then walked out. I was able to get the meds and said "here, you have to take these" put them in her hand and down they went. Its a shame when I have to do their job. I needed to speak to someone prior to getting there, but no answer. As i pulled in i made the call again, again no answer. I went in cautiously to see two nursing staff members standing behind the desk. I made a third call to watch both look at a ringing phone. No answer. She sat in dirty undergarments that never got changed and she had to be taken to ER twice for UTI's. Done with it all, she needed moved. Think twice before putting your loved one here.
