I went there to visit a good friend since elementary school. The receptionist and nurses were very friendly. His room had 3 beds, all of which were occupied, so it was pretty close quarters. This was in the dementia wing. I must say it was a little depressing. I didn't stay to see what they were having for lunch.

It took me a couple of months to put a review online. When I complete a review, I attempt to make it as objective, honest, and as accurate as possible based upon my interaction with staff. It took me a couple of months to put a review online. When I complete a review, I attempt to make it as objective, honest, and as accurate as possible based upon my interaction with staff. I am the court-appointed legal guardian for my mother, who has been a resident at LifeCare of Westlake since April 2026. Unfortunately, our experience has been both good and disappointing. My first impression of this facility is that is great! I've wanted my mom at this facility for a while. I've dealt with several facilities and hospitals in the past, now I'm becoming very knowledgeable dealing with nursing home facilities or skilled care facilities. The receptionists are great! Marty , the admission Director is great. Along with some of the staff, Kenton, Barbie, Tasha, Diamond, Melissa, Cordelia, Jennifer, Lorie, Barb, the Nurse, etc.. These are some if the staff that I applaud! My mom began her stay on the dementia unitI April , 2026. I visited everyday or every other day. My mom was changed. However, there were times , she was left in the dining area all day sometimes in a soiled depend. I had issues with her being taken into the dining room and having to sit all day. Next, we experienced clothes or property that was brought up being left in the closet. Whomever, the STNAs were not taking the clothes down to be labeled. In fact, She still has a missing bag of clothes. My mom experienced an unwitness fall within the first two weeks of being on the dementia unit. I was contacted by a nurse who mentioned that all STNAs were out taking garbage. Two days later, my mom experienced an alleged physical abuse by an STNA. My mom was shoved into the shower with a cap on her head and her face was shaved against her will and consent without her consent. I am the court appointed guardian, I was never informed about the incident. My mother told me when I came to later that day to visit. She was very disgruntled. I was very peeved about the incident and demanded and investigation by the administrator and the Director of Nursing. As a result, the Westlake Police department was called, police report filed, complaint to the Ohio department of Health, the ombudsman office, and a complaint against the person's license. My mother was moves on a regular unit.I observed repeated issues with personal hygiene and basic care. To the point where I spent $479.89 out of pocket for personal hygiene items and hemorrhoid cream for gastro bleeding. To which the facility has failed to reimburse me. There were multiple occasions when my mother was left in soiled or dirty clothing and in need of assistance. Her call light would be on for 25 to 30 minutes without assistance, skipped meds, no water or ice, etc.. I also became concerned about wounds that developed on her leg and whether appropriate preventative and wound care measures were consistently being followed. I was especially troubled by what I considered unprofessional comments and conduct from members of the nursing leadership team. As a guardian advocating for my mother's care, I expected concerns to be addressed professionally and respectfully. My mother experienced three falls while residing at the facility, raising serious concerns about supervision and fall-prevention measures. In addition, there were missed appointments and communication issues that created further frustration and anxiety for our family. Sending a dementia patient to the doctor alone, missing paperwork from doctor's visits, Failure to communicate Dr Summers or doctor visits with family. Families place enormous trust in nursing facilities to provide safe, dignified, and compassionate care. Based on our experience, If, you place someone at this facility, you have to check on them especially if you are the legal guardian or POA. I encourage anyone considering placement here to closely monitor their loved one's care and document concerns promptly. While staff seem well meaning, I am deeply concerned about the cleanliness of the rooms doing my visit.

Everyone has been so helpful. Especially Eric in the memory care unit has been so patient and kind. This nursing home definately needs structural updates but the workers are outstanding.

Workers seem very nice and helpful. Kimmi is happy and content to stay there, and willing to come back, when taken out for a few hours. Food tray could be improved. A cheeseburger with no condiments. She ordered a quesadilla. Desserts could be better. A half bowl of chopped pears ? A strawberry shortcake or piece of cake would be more exciting .

I firmly believe that Life Care is the best rehab and long term nursing facility i have dealt with. The director and administrative staff are compassionate, hard-workers. The director is amazingly compassionate and hands-on. They are not money oriented. I would trust them to have my back as i do theirs.

My dad’s brief stay in the memory care unit for rehab was a mixture of both good and bad. The room he stayed in had 3 beds, and although he ended up being alone in there for his stay, it’s pretty crazy sticking 3 people with dementia in the same room. His tv was broken and had a fuzzy picture, which is a pretty significant portion of entertainment for folks with dementia, so it’s a bummer his one source of entertainment in his room was busted. His bathroom had a broken light, which caused him to either pee with the door open and miss the toilet or get confused and pee in the sink in the room. When I stopped in, I would go in there and wipe down the toilet because he said it was wet and didn’t understand why. The staff was a mixture of both kind and not so kind. He would point out which of the staff was being mean and yelling at patients when there were no visitors. But then there was some very nice staff who kept the patients entertained with group activities, communicated well with me about his progress, and talked easily with the patients. Walking through the halls, the vibe of the whole place is relatively relaxed, but kind of sad. Most people are pretty ill so many folks spend a lot of time resting in their rooms, but the halls also have plenty of residents walking around chatting with each other and staff, or hanging out in the dining areas eating and socializing. For hygiene, he had fresh set clothes on daily and he was given a shower when he was there (1-2x week), but he wasn’t able to brush his teeth every day, shave that often, or cut his nails. So overall, it was not the best experience, but not the absolute worst either. He was mostly safe, somewhat clean, fed, and had access to socializing with others. He had both kind and mean staff around him. His room was pretty disappointing.

The staff is friendly. But the facility is old and leaves somethingto be desired.

Extremely kind, professional and caring - from the outgoing, hard-working office/management staff - to the doctors and incredible nursing team, the aides, just everyone - this is a wonderful, very compassionate facility. There's an authentic feeling of total teamwork to be found here. Absolutely amazing and so very much appreciated!
